Our Duties
MyHealthSolutions is required by law to maintain the privacy of your Protected Health Information (PHI), to provide you with this Notice of our legal duties and privacy practices, and to follow the terms of this Notice currently in effect.
How We May Use and Disclose Your Health Information
Treatment: We may use and disclose your PHI to provide, coordinate, or manage your healthcare and related services. This includes sharing information with your physicians, nurses, and other care providers involved in your RPM or CCM program.
Payment: We may use and disclose your PHI to obtain payment for services, including submitting claims to Medicare, Medicare Advantage, and PPO insurance carriers.
Healthcare Operations: We may use and disclose your PHI for operational purposes including quality assessment, training, compliance, and accreditation activities.
As Required by Law: We will disclose PHI when required by federal, state, or local law, including public health reporting and law enforcement requests as legally required.
Your Rights Regarding Your Health Information
Right to Inspect and Copy: You have the right to inspect and obtain a copy of your PHI maintained in our records.
Right to Amend: If you believe information in your record is incorrect or incomplete, you may request an amendment.
Right to an Accounting of Disclosures: You have the right to request a list of disclosures we have made of your PHI, other than for treatment, payment, and operations.
Right to Request Restrictions: You may request restrictions on certain uses and disclosures of your PHI.
Right to Confidential Communications: You may request that we communicate with you in a specific way or at a specific location.
Right to a Paper Copy: You have the right to a paper copy of this Notice at any time.
Complaints

Effective Date
This Notice is effective January 1, 2025. We reserve the right to change this Notice and to make the revised Notice effective for medical information we already have about you as well as any information we receive in the future.
